Deliberations at the Commission of Inquiry on State of Capture in South Africa under the then Deputy Chief Justice Zondo laid bare how lack of consequence management as accountability mechanism for good governance was compromised in the South African public sector institutions and entities. Problems with governance and management of state entities such as ESKOM and TRANSNET are compounded by problems of corruption, fraud, and mismanagement of public resources by board members and senior managers, which cascades down to junior public officials.
